Insurance and Motor Vehicle Accidents:


I am currently NOT taking medical insurance, motor vehicle accidents, or workman's comp.


Discounts: Everybody likes to save money!

  • Only one discount per person, for that person, but that discount may be used for multiple massage appointments. 
  • Discounts do not apply to Bundle Package Purchasing.
  • Do to the extra set-up time, discounts do not apply for prenatal massages.
  • Available discounts are as follows:


  1. Senior Discount: must be over 65 and be able to show drivers license if necessary. If purchasing online use discount code: WISEOWL for $5.00 off at checkout
  2. Military Discount: active or retired and be able to show military or veteran ID card.  If purchasing online use discount code: USAPRIDE for $5.00 off at checkout
  3. Referral Discount: if a client chooses to refer 5 friends and they come in with that referral and purchase a massage. This discount can only be applied by massage therapist.

Cancellation: Hey it (work, kids, LIFE) happens!


For a no charge and no penalty client must cancel/reschedule appointment themselves online OR notify massage therapist by, call, text, or email 24hrs before appointment time.


Notifying with less than 24 hrs, client gets ONE pass without penalty, however if there is a second occurrence of less than 24hr notification of cancellation/rescheduling, client must pay a cancellation fee equaling 50% of the cancelled/rescheduled appointment. This must be redeemed before any future appointments can be made and cannot be used as credit.


If a client has not shown to their appointment time by 5mins passed their scheduled time they will receive ONE courtesy phone call (no call will be made for chair massage appointments), on whatever number has been provided, therefore it is important to update this information when it changes. If there is no response and client has not shown up 15mins (5mins for chair massage appointments) passed their scheduled time, the appointment will automatically be cancelled and be considered a NO PASS or “NO SHOW” situation and will be recorded as such in clients file. The penalty is the client must pay 100% for the appointment and will be banned from making online appointments. 


After 2 cancellations OR reschedules in a row, regardless of amount notice before cancellation, client will lose online scheduling privileges and will have to pay a fee of 20% of the service(s) being canceled or rescheduled for the inconvenience put on the massage therapist.


Any fees accrued will be invoiced to the client via email or mail and must be paid in full online or in person before further appointments can be made or online scheduling can be lifted.


Clients who have used their ONE free pass, have too many reschedules on file, or have a NO SHOW in their file can redeem another free one pass and online scheduling after SIX consecutive ON TIME, AND NO RESCHEDULING/CANCELING appointments.

Minors Seeking Massage:

  

Clients under the age of 17 must be accompanies by a parent or legal guardian during the entire session. Informed written consent must be provided by parent or legal guardian for any client under the age of 17. 


Terms and Conditions to Receive Massage:

  

I give permission to receive massage therapy. I understand that the massage I receive is provided for the basic purpose of relaxation and relief of muscular tension, NOT of a sexual nature and that draping will be used during the session- only the area being worked on will be uncovered.

If I experience any pain or discomfort during this session, I will immediately inform the therapist so that the pressure and/or strokes may be adjusted to my level of comfort. I further understand that massage should not be construed as a substitute for medical examination, diagnosis, or treatment and that I should see a physician, chiropractor or other qualified medical specialist for any mental or physical ailment that I am aware of. I understand that massage therapists are not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illness, and that nothing said in the course of the session given should be construed as such.

Because massage should not be performed under certain medical conditions, I affirm that I have stated all my known medical conditions, and have answered all the question honestly. I agree to keep the therapist updated as to any changes in my medical profile and understand that there shall be no liability on the therapist’s part should I fail to do so.  


  

Bundle Package Terms and Conditions

Terms and conditions that purchaser/client must agree to the following when purchasing a bundle package. The intended purpose of a bundle package is that the client will be coming in at minimum, every two weeks for 60 minute session to focus on a specific issue or for maintaining physical wellness. With this in mind, it is therefore important to know that these massage credits when purchased in a bundle package, must be used within a timely manner. Clients must use the 4  60 minutes of massage credit within 10 weeks after the purchasing date. After which any remaining credits that are unused will be expired and unusable to the client. 

To ensure that client uses credits, it is required that appointments be scheduled at time of purchase or within one week of purchase via online. Rescheduling is acceptable, however client must still adhere to the 10 week policy. Upon the approval of the massage therapist, there may be cases for a "hold", however, it will be up to the client to notify the massage therapist of the particular situation such as family emergency or medical situation that would prohibit a client from using their credits. A date must then be agreed-upon when the client will be able to resume their massage sessions. 

It is not the responsibility of the massage therapist to ensure that the client is using their credits. Massage therapist will however notify client if credits are in danger of expiring within two weeks of the date via text, email, or by phone call. If massage therapist has not received reply or has client has not made an appointment online, no secondary warning will be issued and the remaining credit will be in danger of expiring, regardless of therapists availability, with no responsibility put on the massage therapist. Once expired credits cannot be used in anyway shape or form to the client. In the case where a client cancels with less than 24hrs, reschedules 2 times in a row, or no-shows for an appointment, cancellation policy will be enacted and credits will be used toward that policy. Credits cannot be transferred to another person, or be transferred to a gift certificate. Lastly there are absolutely no refunds once purchased.
